Use of the AngioJet system in the treatment of acute STEMI

The authors present the case report of a patient with acute myocardial infarction with ST elevations (STEMI), treated with direct percutaneous coronary intervention (PCI) using the AngioJet system. Acute thrombotic occlusion of the distal part of the right coronary artery (RCA) was diagnosed by urgent selective coronary angiography. As balloon predilatation failed to restore, at least partly, blood flow in the coronary artery, 20 mg of intracoronary abciximab (ReoPro) was given, again without any improvement in blood flow. Consequently, the AngioJet system was used, followed by blood flow restoration to TIMI III after thrombus aspiration from the distal RCA, without the need for stent implantation. There were no complications in the ensuing post-procedural period, with the patient discharged home on the fifth day.
Thrombectomy with the use of the AngioJet system following PCI is an alternative treatment in patients with STEMI and the presence of a large thrombus.
