Extra-anatomical bypass with Bentall's procedure in the treatment of complicated recoarctation of the aorta

In their case report, the author present a patient with complicated recoarctation of the aorta managed by extra-anatomical bypass from the ascending aorta to the descending thoracic aorta. The bypass was performed simultaneously with Bentall's procedure from midline sternotomy. The posterior pericardial approach was used to gain access to the descending aorta.
The indication for the procedure was recoarctation of the aorta in a patient with ascending aorta aneurysm and aortic valve regurgitation.
Despite a complicated early postoperative course, the patient was discharged after 2 months to receive home care. Aortic angiography performed at 16 months showed patent reconstruction with a zero gradient between the ascending and descending aortas. The patient has been free of problems for 30 months post surgery.
