Acute and chronic heart failure remains a very serious and high priority problem in contemporary cardiology. New, modern drugs are currently emerging in the treatment of heart failure while the possibilities of their further use for this diagnosis are being expanded. These findings have been summarized in the 2023 update of the European Society of Cardiology Guidelines for heart failure. This article provides a brief summary of the new developments in pharmacotherapy based on the above recommendations.

A lack of adherence is associated with a worse prognosis. Decreasing adherence in post-MI patients leads to an increased risk of recurrence of ischemic events. In addition, due to the aging population and improved quality care for ACS with better survival, there is an increase in the number of patients who need more personalized secondary preventive interventions. For these patients, improving adherence is another way to improve their prognosis. Atrial fibrillation (AF) is the most common cardiac arrhythmia in adults with substantial morbidity and mortality. AF increases the risk of stroke 5-fold compared to the general population and oral anticoagulant therapy represents the cornerstone for stroke prevention. Therefore, it is necessary to identify predictive factors for the onset of AF in order to screen patients with a high probability of AF development. We report the case of a 69-year-old patient asymptomatic for palpitation with a 24-hour Holter ECG showing several ncPACs (non-conducted premature atrial complexes) leading to AF onset.

Background: Exercise stress testing (EST) can cause false-positive diagnoses of myocardial infarction (MI) in individuals with Wolff-Parkinson-White (WPW) syndrome. Case: A 30-year-old male had a history of palpitation and near syncope 2 months before examination. The pre-test probability (PTP) score was 1% and baseline ECG showed typical features of WPW syndrome. Additionally, risk stratification was performed with EST using the Bruce protocol. Cardiac myxoma is the most common primary tumour of the heart. We present a case of a 46-year-old man with a 3-month history of progressive fatigue, exertional dyspnoea, fever, and elevated inflammatory markers. Echocardiography revealed left atrial tumour. The patient underwent successful surgical resection of the tumour; the histological study of the mass confirmed the diagnosis of myxoma.

Introduction: This study investigated the relationship between the subjective New York Heart Association (NYHA) class and objective exercise capacity (VO2peak) in patients with heart failure and whether it is influenced by medication adherence.
