Introduction:The clinical diagnosis of death may be inaccurate after cardiac surgery. Autopsy is an important source of knowledge and an integral part of medical education. The only true results can be obtained by comparing the two procedures. Methods:Data of all in-hospital deaths after cardiac surgery, occurring in a single institution between February 2002 and December 2004, were collected prospectively. Clinical diagnoses were compared with the autopsy findings. Results:Between February 2002 and December 2004, a total of 2,613 patients were operated on. During this period, 69 deaths occurred. Autopsy was...

Aim of study:To evaluate the long-term results of Senning procedure for transposition of the great arteries (TGA) more than 5 years after surgery with emphasis on cardiovascular function, physical activities and quality of life. Patients:168 patients operated on between 1984 and 1997 were followed up. The age at surgery was from 1 month to 9 years (median 6 months). In 35 patients with complex TGA associated ventricular septal defect was closed or pulmonary artery stenosis released. Sixteen (9.5%) patients died at an interval of 2 months to 20 years (median 13 years) postoperatively. A total of 152 patients had detailed clinical...

Introduction:Orthotopic heart transplantation (OHT) is currently considered standard treatment of patients with end-stage heart failure. The program of OHT in the Czech Republic was launched at the Institute for Clinical and Experimental Medicine (IKEM) on 31 January 1984 and, in October 2005, the first transplant recipient marked 21 years since his successful surgery. This presentation is an analysis of the benefits and limitations of the program of OHT at IKEM. Methods:Our group of patients includes 572 (480 males and 92 females) patients undergoing OHT; four males had repeat transplantation. The mean age of our patients...

The etiology and pathogenesis of pulmonary arterial hypertension

Cor Vasa 2006, 48(3):108-113 | DOI: 10.33678/cor.2006.036
The term pulmonary arterial hypertension (PAH) refers to idiopathic and familiar pulmonary hypertension, furthermore to PAH in collagen vascular disease, PAH in congenital left-to-right shunt defects, PAH in portal hypertension, PAH in HIV infection and after anorectic drugs or toxic agent abuse. The body of our knowledge regarding the pathogenesis of PAH has expanded dramatically, as reflected in novel therapeutic modalities.PAH is a condition associated with proliferation (of smooth muscle and endothelial cells) and vascular occlusion due both to thrombosis and proliferative changes.Remodeling may be triggered by mechanical (vascular wall...

Reverse cholesterol transport (RCT), i. e., transport of cholesterol from peripheral tissue to the liver, is an important physiological mechanism whereby a major role is played by high-density lipoprotein (HDL) particles. Removal of excess cholesterol from cells is important for maintaining cholesterol homeostasis in cells and occurs via several mechanisms.Cholesterol is removed from cells actively mainly via membrane proteins called ABCA1 (ATP-binding cassette transporter A1) or ABCG1 and ABCG4 (ATP-binding cassette transporters G1 and G4); additional cholesterol may leave cells via SR-BI (scavenger receptor class B type I) or diffusion. The relative...
